ASUHAN KEPERAWATAN GAWAT DARURAT PADA Tn.O DENGAN GANGGUAN SISTEM PENCERNAAN : ILEUS OBSTRUKSI DI RUANG INSTALASI GAWAT DARURAT RUMAH SAKIT HASAN SADIKIN BANDUNG

Penyakit saluran cerna tergolong 10 besar penyakit penyebab kematian didunia. Ileus adalah gangguan aliran normal isi usus sepanjang saluran usus. Penyebab ileus obstruksi berkaitan pada kelompok usia yang terserang dan letak obstruksi, 50% terjadi pada kelompok usia masa dewasa akhir (36-45 tahun) dan lansia awal (46-55 tahun) akibat perlekatan oleh pembedahan sebelumnya. Tumor ganas dan volvulus merupakan penyebab tersering obstruksi usus besar pada usia dewasa akhir. Oleh karena itu tujuan dari penulisan ini untuk mengetahui asuhan keperawatan pada pasien dengan gangguan system pencernaan : Ileus Obstruksi meliputi pengkajian, diagnosa, intervensi, implementasi dan evaluasi. Setelah dilakukan tindakan keperawatan selama 2 hari ditemukan 6 diagnosa diantaranya defisit nutrisi, resiko ketidakseimbangan cairan, pola nafas tidak efektif, konstipasi, hipertermi, dan hipotermi. Intervensi yang dilakukan yaitu memberikan nutrisi parenteral, mengurangi muntah, memberikan terapi oksigen, mengidentifikasi penyebab tidak bisa BAB, memberikan kompres hangat, melakukan penghangatan pasif. Implementasi telah dilaksanakan sesuai intervensi yang ditetapkan. Setelah dilakukan evaluasi didapatkan hasil bahwa hanya satu diagnosa keperawatan yang teratasi yaitu hipertermi dan satu diagnosa keperawatan yang teratasi sebagian yaitu pola nafas tidak efektif. Dengan adanya karya tulis ilmiah ini diharapkan dapat menambah ilmu pengetahuan tentang ileus obstruksi dan menjadi referensi untuk penelitian selanjutnya.

Digestion systems disorders is classified as the top 10 causes of death in the world. Ileus is a disorder of the normal flow of intestinal contents along the intestinal tract. The causes of obstructive ileus relate to the affected age group and the location of the obstruction, 50% occurring in the late adulthood age group (36-45 years) and early elderly (46-55 years) due to adhesions by previous surgery. Malignant tumors and volvulus are the most common causes of colon obstruction in late adulthood. Therefore, the purpose of this paper is to determine nursing care in patients with digestive system disorders: Ileus Obstruction includes assessment, diagnosis, intervention, implementation and evaluation. After nursing action for 2 days, 6 diagnoses were found including nutritional deficits, risk of fluid imbalance, ineffective breathing patterns, constipation, hyperthermia, and hypothermia. The interventions carried out were providing parenteral nutrition, reducing vomiting, providing oxygen therapy, identifying the causes of unable to defecate, providing warm compresses, performing passive warming. Implementation has been carried out according to the specified interventions. After the evaluation, the results showed that only one nursing diagnosis was resolved, namely hyperthermia and one nursing diagnosis that was partially resolved, namely the ineffective breathing pattern. With this scientific paper, it is hoped that it can add knowledge about obstruction ileus and become a reference for further research.
