ASUHAN KEPERAWATAN PADA TN. A DENGAN GANGGUAN PERSEPSI SENSORI: HALUSINASI PENDENGARAN DAN PENGLIHATAN DI KLINIK UTAMA KESEHATAN JIWA NUR ILAHI BANDUNG v + 147 halaman + 9 tabel + 1 bagan + 1 gambar + 13 lampiran Gangguan jiwa disebut sebagai kondisi perkembangan perilaku, afek, biologis, psikologis, sosial yang tidak sesuai pada individu. Indikator kesehatan jiwa yang dinilai antara lain gangguan jiwa berat (Halusinasi, ilusi, waham, gangguan proses pikir, kemampuan berpikir, serta tingkah laku aneh) dan gangguan mental emosional (Distress psikologik). Hasil rekam medik Klinik pada bulan Januari-Desember tahun 2018 tercatat bahwa jumlah pasien rawat inap dan rawat jalan sebanyak 7.610 orang, kasus skizofrenia dengan halusinasi sebanyak 1.276 orang. Halusinasi merupakan persepsi sensori terhadap stimulus internal, dapat mempengaruhi indra pendengaran, penglihatan, taktil, gustatory, penciuman dan kinetic. Tujuan penulis mampu menggambarkan Asuhan Keperawatan pada klien Skizofrenia dengan Gangguan Persepsi Sensori: Halusinasi pendengaran dan penglihatan di Klinik Utama Kesehatan Jiwa Nur Ilahi Bandung. Metode yang digunakan asuhan keperawatan meliputi pengkajian, menentukan diagnose, merencanakan tindakan, melakukan tindakan dan evaluasi. Selama 4 hari pertemuan dengan klien, telah dilakukan asuhan keperawatan sesuai dengan rencana keperawatan gangguan persepsi sensori: halusinasi dapat dikontrol dengan cara mengenal halusinasi, menghardik, bercakap dengan orang lain, melakukan aktivitas positif menyibukan diri sendiri dan patuh minum obat teratur. Saran sebagai pemberi asuhan keperawatan kepada klien perlu pendekatan dengan komunikasi terapeutik, menjalin hubungan saling percaya, dan bina trush, untuk mendorong klien lebih terbuka dan kooperatif dan tercapai rencana yang akan diberikan. Peran perawat dan kehadiran keluarga klien sangat penting dan berikan Reinforcement positif akan meningkatkan harga diri klien.

The mental health indicators are considered among others severe mental disorders (hallucinations, illusions, insight, disruption of thought processes, thinking abilities, and strange behavior) and mental emotional disorders (distress psychology). The result of medical records in January-Desember 2018 noted that the amount of inpatient and outpatient were 7.610 patients, schizophrenia cases with hallucinations were 1.276 patients. The hallucinations are sensory perceptions of internal stimuli, can affect the senses of hearing, vision, tactile gustatory, smell and kinetic. The writer's purpose is to describe Nursing Care on Schizophrenia patient with the disruption of sensory perception : Hallucination of hearing and sight in Main Clinic of Mental Health Nur Ilahi Bandung. The method used for nursing care is assessment, determine the diagnosis, action planning, do an action and evaluation. During the 4 days meeting with the patient, the writer has been done the nursing care in accordance with nursing plan of the disruption of sensory perception : hallucination can be controlled by knowing the hallucination, rebuke, talking with other people, doing positive activity like concerning themselves and take medication regularly. The suggestion as the nursing caregiver to the patient is it needed the theurapeutic communication approaches, establish a trusting relationship, and building trust, for encouraging the patient to be more open and cooperative and achieving the plan that will be given. The role of the nurse and the family is so important and give the positive reinforcement will increase the patient's self-esteem.
