ASUHAN KEPERAWATAN PADA PASIEN NY. S (36TH) P2A1 POST PARTUM SPONTAN HARI KE-1 DI UPT PUSKESMAS PUTER KOTA BANDUNG

S (36 TH) P2A1 POST PARTUM SPONTAN HARI KE-1 DI RUANG BERSALIN UPT PUSKESMAS PUTER KOTA BANDUNG v + 52 halaman + 5 tabel + 4 lampiran Asuhan keperawatan ditujukan kepada individu dan keluarga, termasuk diantaranya pada ibu Post partum. Selama Post partum perlu mendapat perhatian, dikarenakan angka kematian ibu 60% terjadi pada Post partum. Hasil laporan rekam medik di UPT Puskesmas Puter Kota Bandung tahun (2018) jumlah ibu Post partum yang mengalami komplikasi dan tertangani sebanyak 714 orang, dan pasien yang dirujuk Rumah Sakit berjumlah 237 orang. Tujuan penelitian ini adalah menggambarkan asuhan keperawatan pada pasien Ny.S dengan Post partum Spontan Hari ke-1. Metode desain yang digunakan pada penelitian ini adalah studi deskriptif laporan stadi kasus, dengan sampel 1 orang ibu Post partum. Hasil penelitian setelah dilakukan asuhan keperawatan berdasarkan diagnosa yaitu ketidaknyamanan pasca partum berhubungan dengan luka episiotomi, resiko infeksi berhubungan dengan terbukanya sistem barier tubuh, menyusui efektif berhubungan dengan keadekuatan suplai ASI. Dapat disimpulkan hasil dari pengkajian, diagnosa, perencanaan, tindakan keperawatan, evaluasi dari hasil nyeri dan ketidaknyamanan Post partum sudah teratasi. ASI efektif dengan keadekuatan suplai ASI. Diharapkan Puskesmas dapat memberikan asuhan keperawatan secara komprehensif, sehingga pelayanan kesehatan yang diberikan dapat berupaya optimal dan mendukung kesembuhan klien. The purpose of this study is to describe nursing care in patients with Mrs.S with 1 st day spontaneous Post partum. The design method used in this study was “descriptive study of case study reports, with a sample of 1 mother spontaneous Post partum.the results of thr study after nursing care based on the diagnosis of Post partum discomfort associated with episiotomy wounds, the risk of infection associated with damage to the integrty of the skin, effective breastfeeding associated with adequate breast milk supply. It can be concluded the results of assessment, diagnosis, planning, nursing action, evaluation of the results of pain and post partum discomfort int the patient has been resolved.
